Please read the instructions below carefully before registering and follow
the payment instructions. You may register via the Internet or by fax or e-mail.

to be fully completed and returned by e-mail or fax, along with a
copy of the bank transfer, to:
Confirmation will be sent upon receipt of payment. In order to receive e-mail
confirmation, an e-mail address must be provided on the registration form.

Registration for events covered by the registration fee will only be confirmed
if you tick “yes” on the registration form. Please follow the payment instructions
for all fees carefully.

IMPORTANT REMARK: Please be aware that presentation of any accepted abstract is conditional to the registration of at least one author.

MLTD Members
To benefit from MLTD membership discount, please indicate your membership number on the
registration form.
Students
Medical students or fellows in training under the age of 35 are eligible to
pay student registration fees. Student status must be certified by a copy of a student ID card,
to be submitted with the registration form. Student ID must be shown upon badge collection at
the congress reception desk for verification.
The registration fee for delegates covers:
• Admission to the congress and the exhibition
• Congress kit (Programme, Book of Abstracts, Badge and Congress bag)
• Congress documentation (Attendance certificate, CME certificate for Italian and European delegates)
• Daily lunch and tea/coffee
• Opening ceremony and welcome reception
Payment
Payments must be made in advance, either by credit card or bank transfer.
We accept Visa or Eurocard/Mastercard. Please provide card number, expiry date and CVV code
on the registration form.

Bank transfers must be made before 25 June 2010. Admission to the congress is granted
only if Arseducandi has received the registration fee. Delegates who have made late
payments should bring proof of payment to the congress.
Name Change
Once registered, if you are unable to attend the congress due to illness or unforeseen
circumstances, you may request that a colleague takes your place.
A fee of EUR 60.00 will be charged for changing the name of the delegate.
Please advise the Secretariat thereof by fax or e-mail.
Names of delegates may be changed until 8 June 2010.

The registration of groups of 20 people or more cannot be done online, but

Cancellations & Refunds
Notification of cancellation must be sent in writing to:
Cancellations will be accepted until 6 May 2010. Registration fees will be refunded
in full, less a cancellation fee of 20% of the total. We regret that no refunds may
be provided for cancellations after 6 May 2010.

If you require a visa to enter Italy and you need a letter of invitation for this
purpose, after you have completed registration, please send a request to:
